Medical Insurance
Humana plan with PPO network. Employees may elect employee
only, employee and spouse, or family coverage. Employee
contribution and deductibles vary by plan.
Prescription Insurance
Included with Humana plan. Three deductible levels, Generic,
Preferred and Premium. Mail order is available.
Dental Insurance
Guardian plan with PPO network. Deductibles vary with plan
selected.
Vision Insurance
Guardian plan using Davis Vision.
Group Life and AD&D Insurance
Employee life and accidental death and dismemberment insurance
is provided to all full-time company employees. Benefit
varies by job class.
Short Term Disability Insurance
This benefit provides income while an employee is absent from
work due to non-occupational injuries and illnesses. Limitations
on weekly amount and number of weeks. Is included when employee
elects medical insurance coverage.
Flexible Spending Accounts
Permits employees to elect amounts to be deducted from their
paychecks to pay for unreimbursed medical costs or child care
expenses. Employees may also elect to purchase various plans
from sponsor AFLAC that are paid via payroll deduction.
401(k) Plan
Provided by The Principal Group. Employees may elect to contribute
to this retirement plan via payroll deduction. Company paid match
in the amount of 25% of the first $2,000 contributed by the employee
each year. Contribution amounts and vesting limited by plan provisions
and IRS regulations.
